Blood Service agreements are used for hospitals receiving and returning blood products. The agreement and published fees are for full service hospitals. Full service hospitals are supplied with blood and blood components exclusively by LifeShare Blood Centers.
Return privileges are described in the routine Blood Service Agreement. In compliance with LifeShare’s Quality Plan, facilities returning blood products for credit are subject to at least an annual audit by blood center staff of blood storage equipment and records.
Compatibility testing agreements are used when Laboratory Services of LifeShare Blood Centers provides compatible blood for facilities to transfuse to patients. The Compatibility Testing Agreement does not routinely describe (and therefore disallows) return privileges. Should a facility consider that they have equipment and storage conditions which should be considered for return privileges they may contact their local center to arrange for an audit. Successful completion of an audit is necessary before the agreement can be amended to allow returns. |
