Service Agreements
Blood Service agreements are used for hospitals receiving and returning blood products. The agreement and published fees are for full service hospitals. Full service hospitals are supplied with blood and blood components exclusively by LifeShare Blood Centers.
Return privileges are described in the routine Blood Service Agreement. In compliance with LifeShare’s Quality Plan, facilities returning blood products for credit are subject to at least an annual audit by blood center staff of blood storage equipment and records.
Compatibility testing agreements are used when LifeShare Blood Centers’ Laboratory Services provide compatible blood for facilities to transfuse to patients.
The Compatibility Testing Agreement does not routinely describe (and therefore disallows) return privileges. A facility may request an audit of its equipment and storage conditions to be considered for return privileges.
Contact your local center to arrange for an audit. Successful completion of an audit is necessary before the agreement can be amended to allow returns.
