• Prescriptions are valid for a maximum of 12 months. Orders or modifications may require LifeShare’s Medical Director’s approval or consultation.
• Donors may come in less frequently than but not more frequently than indicated above unless approved by LifeShare’s Medical Director.
• Orders should be submitted 72 hours before the first collection to allow adequate time for review and data entry.
• Depending on location, therapeutic phlebotomy may only be offered at specific times or on specific dates. Donors should schedule appointments at collection site for Therapeutic Phlebotomy.
• Depending on diagnosis, a $75 service fee may apply.
